New Patient Chiropractic Exam + Adjustment

Welcome and thank you for choosing Pain Free Chiropractic! Please print, fill out, and bring the new patient paperwork in with you on your first visit. If you cannot do so, please arrive 20 minutes early to your appointment to fill out the paperwork. Also please bring your ID and insurance card with you. We look forward to getting rid of your pain and keeping you healthy!

Includes new patient history, neurological exam, orthopedic exam, chiropractic exam, consultation, and adjustment.

Reiki is a Japanese healing practice that involves channeling energy through the hands to promote relaxation, balance, and healing in the body.* ** ***

*After reiki you will have the ability to get adjusted if you choose. You do not have to book separately.

**Please only schedule reiki on the hour or half hour.

***All reiki time includes 5-10 minutes to allow you time to undress, redress, and change the sheets for the next patient.

I had a deep tissue massage by Mariamma, she was amazing , very intentional. Next time I will have to do the 60 min. Definitely recommend !

Massage: Jennel was a very experienced, and thorough professional. She knew exactly what I needed when I shared my pain and was able to identify what I’ve been through via examination. I had more range of motion in my neck and shoulders after getting up from her table. 10/10 recommend.
